Our ability to provide post-acute rehabilitation, long term skilled nursing care, memory care and respite services allows us to assess and ensure the best care for our clients.

Post-Acute Rehabilitation

After surgery, trauma or illness, extra healing on your journey from the hospital to home may be just what the doctor ordered. Our post-acute care programs are individualized and include physical, occupational and speech therapy while allowing focused time to heal. Our team works to return you to the highest degree of health and wellness. Private rooms and common spaces allow families to visit and actively participate in your rehabilitation.

Skilled Nursing

When extended care is needed, our skilled nursing team provides continuing restorative care. Promoting functional independence, our holistic team addresses physical, psychosocial and spiritual needs. Providing the highest level of care outside the hospital, trained staff gently deliver medical care and assistance with bathing, eating, toileting, dressing and getting up and down safely. With music therapy, recreation therapy, transportation services and round the clock care, we bring more to the lives of our clients each day.

Memory Care

When cognitive challenges impact the safety of a person with memory loss, our safe and secure environment is designed to address all their needs. Staff experienced in dementia care ensure our care respects the changing lives of our clients and allows them to thrive and feel secure. The addition of twice daily music therapy provides extra structure and support to help them navigate their day.

Respite Care

No matter how much you love or care for someone, caregiving can be a lot of work, especially when you have other roles or responsibilities to fulfill in your life. When temporary relief from the responsibilities of caregiving are needed, our respite care program provides a much-needed break from the demands of caregiving for an aging or disabled family member. You can come back refreshed and re-energized, while the interaction with new people can help minimize social isolation for your loved one.

Music Therapy

Music Therapy is an established health profession in which music is used to address individuals’ physical, emotional, cognitive and social needs. Our board-certified music therapy coordinator has led our program since 1996, enhancing the lives of our residents while also providing internship opportunities for students in the field. With a daily schedule, residents have opportunity for group and individual music therapy activities. Based on studies that music therapy benefits those with dementia, twice daily activities occur on our Memory Care unit.

Recreation Therapy

Recreation Therapy provides a variety of therapeutic services which include assessment, program planning, program implementation and evaluation.  They bring expertise in evidence based practice, program development and evaluation, outcome measurement and group facilitation that support and promote the integration of new, non-pharmacological interventions into the day to day care of our residents. Person-centered care and engagement in meaningful activities are the foundation for reducing anti-psychotic medications, supporting positive behavioral responses and supporting social engagement. Their techniques further support persons with dementia in living a life that is meaningful and with sense of purpose and well-being.
